Feedback templates

Create and edit Feedback 360 questionnaires in Feedback › Templates: chapters, paragraphs, open questions, ratings, multi-choice, required questions, tags and translations.

A template is the questionnaire reviewers answer in a Feedback 360 campaign. Templates live in the company library at Feedback › Templates and can be reused in as many campaigns as you want. When a campaign is launched, the template is copied into the campaign, so editing a template later never changes a campaign that is already running.

Before you start

  • You need the Manager creator, Scoped admin, Admin or Super admin role. Feedback › Templates is not shown to employees and managers.
  • Manager creators only see the templates they created in the library. Scoped admins, Admins and Super admins see every template of the company. In the campaign wizard, every company template can be selected, whoever created it.
  • Only the template's creator and admins can edit, tag, translate or delete a template; anyone else who opens it sees it as Read-only template and can use Duplicate template.

The template library

Go to Feedback › Templates. The page (subtitle Create and manage feedback templates, to be used in campaigns.) lists templates in a table:

Column Content
Title Template name. Click the row to open it.
Last update Date of the last change.
Questions Number of blocks in the template.
Tags Tags attached to the template.

Use the search field to find a template by title, and the filter button to filter by Last update, Creator, Questions (number) or Tags. The "..." menu at the end of each row offers Edit template tags (creator and admins), Duplicate and Delete (creator and admins).

Create a template

  1. Go to Feedback › Templates and click New template.
  2. A template called "New template" is created and opens in the editor. Changes are saved automatically: the tag at the top shows Saving..., then Saved (or Not saved if the connection failed).
  3. In the header block (Template), fill in the Template name, an optional Description and an optional Video (Youtube) link (shown as an embedded video at the top of the questionnaire). Click the tag icon of the block to add tags.
  4. Click Add (under the header or under any block) and choose the type of block to insert at that position.
  5. Fill in each block (see the types below) and set its switches.

Tip: To start from an existing questionnaire, use Duplicate in the "..." menu of the library, or Duplicate template when you open a template you can't edit. The copy is yours, named "Copy of ..." (in your interface language) and opens straight away in the editor.

Block types

Block (menu label) Used for Settings
Chapter A section title that groups the next questions. Chapters appear in the Chapters side panel reviewers use to navigate. Chapter name, Description.
Paragraph Explanatory text for reviewers. Not answered. Description only.
Open question A free-text answer. Question, description, Required.
Rating A score on a scale, shown as clickable star icons. Question, description, Max rating (from 2 to 10, default 5), Allow comments, Required.
Multi-choice One answer to pick from a list of options. Question, description, Answer options (Add answer option to add more), Allow comments, Required.

Older templates can also contain True/False questions (two options, with editable labels). They still work and can be edited, but new ones can no longer be added.

Required: the reviewer can't submit their review until they have answered this question. Required questions show an asterisk (*) in the form.

Allow comments: adds a free comment field under the answer, so the reviewer can explain their rating or choice.

Chapters and paragraphs are layout only: they are never answered and don't count in completion or exports.

Reorder, duplicate or delete a block

Each block has icons to move it up, move it down, duplicate it (the copy is placed just below, with its options) and delete it.

List icon Opens the Summary panel with the template's blocks, to jump to one.
Eye icon Opens a preview of the questionnaire as reviewers will see it, with a Chapters panel.

Translate a template

If your company works in several languages, you can offer the questionnaire in more than one language. Reviewers then get a Translate interview button on their form to switch language.

  1. Open the template and click the translate icon in the top bar. The Template translation window opens.

  2. Check the Source language row (tagged Source). The link How to translate my template? › Click here opens a guide.

  3. Click Add Language and choose a language.

  4. For each language, use the actions:

    • Translate with AI: OpenAleph translates every title, question and option automatically. The status shows Translating with AI..., and you are notified when it is done.
    • Download translation: downloads a spreadsheet with the texts to translate.
    • Upload translation: imports the filled-in spreadsheet.
    • Delete language: removes this translation.

    Each language shows a status tag (Translation missing, Translating with AI..., Uploading..., Translated), always displayed in English. The workflow is the same as for interview templates, see Translate an interview template and Translating content.

Note: A campaign uses the translations that exist at the time it is launched.

Tags

Tags help you sort a large library. Click the tag icon in the template header, or Edit template tags in the library, then search for a tag or type a new name and create it with New tag. The same tags are used for campaigns.

Delete and restore a template

  1. In the library, open the "..." menu of the template and click Delete, then confirm in Template will be deleted.
  2. The template moves to the bin ("Template has been deleted successfully."). To see it, click the "..." button next to New template and choose View deleted templates. The Deleted templates window lists them with their Deleted on date.
  3. From there you can restore it or delete it permanently (icons on its row).

The confirmation message says deleted templates are stored for 30 days before being permanently deleted.

Consequences:

  • A deleted template can't be selected in new campaigns.
  • A campaign still in draft or scheduled that uses a deleted template can't be launched until you restore the template or pick another one.
  • A template can't be permanently deleted while a draft or scheduled campaign still uses it.
  • Campaigns already launched keep their own copy of the questionnaire and are not affected.

Common questions

I can't edit a template: everything is greyed out.

The tag at the top says Read-only template: you are not its creator and not an admin. Click Duplicate template to get your own editable copy.

I changed a template, but a running campaign still shows the old questions.

That's expected. A campaign copies the template at launch (the participant table shows the template version date). Changes only apply to campaigns launched afterwards.

I don't see "Templates" in the Feedback menu.

Only manager creators, scoped admins and admins see it. Ask your admin if you need to build questionnaires.

As a manager creator, I don't see my colleagues' templates in the library.

That's expected: the library shows manager creators only the templates they created. You can still pick any company template in step 4 of the campaign wizard.

Can I add a True/False question?

Not any more. Use a Multi-choice question with two options instead.
