Create a client survey template
Create, edit, preview, duplicate, delete and restore the questionnaires sent to clients, with Rating, Yes/No and Open questions and the Global rating.
A client survey template defines the questions a client answers. Templates are reusable: the same one can be sent to any number of clients and adapted at any time. They are managed in Client survey › Templates.
Before you start
- Everyone with access to Client survey can see the templates and preview them.
- Creating, editing, duplicating and deleting templates needs the Scoped admin role or above. The New template button and the row menu are shown to everyone, but for other roles the change isn't saved.
- Client survey templates are separate from Team Survey templates.
The Templates page
Go to Client survey › Templates. The list shows:

Search with "Search templates…". The ... menu at the top gives View deleted templates. Each row's menu offers Duplicate, Edit and Delete.
Create a template
- Click New template.
- Under Template information, enter the Template name* (for example "Standard member rating") and an optional Description ("What is this template used for?").
- Under Questions, write each question in Question text… and pick its Question type:
- Rating: the client gives a number of stars. Choose how many in Stars (3 to 10).
- Yes/No: the client answers Yes or No.
- Open question: the client types a free answer.
- Click Add question to add more, and the remove icon to delete one.
- A new template starts with the Global rating question, and the questions you add are inserted above it, so it is always last: a star rating whose text and type can't be changed and that can't be removed. You can only choose its number of Stars. Its score is the one shown on dashboard cards and in the completion email.
- Click Create template.
If a required field is empty, it's highlighted and the template isn't saved. A template without any question can't be saved either ("Add at least one question.").
Note: Every question must be answered by the client before they can move to the next one. There is no optional question.
Preview a template
Click a template in the list. The page shows the questionnaire with a Preview tag: stars for ratings, Yes / No buttons, and a "Your answer..." box for open questions. Back to templates returns to the list.
Edit a template
In the row menu, click Edit, change what you need and click Save changes. Edits apply to surveys sent from now on. Surveys already sent keep the questions they were sent with.
Duplicate a template
Click Duplicate in the row menu to create a copy, for example to make a variant for another type of client.
Delete and restore a template
- Click Delete in the row menu, then confirm in Delete template?. Deleted templates are stored for 30 days before being permanently removed.
- To restore one, open View deleted templates in the ... menu at the top of the page. You can also delete it permanently from there.
Surveys already sent with a deleted template stay readable.
Common questions
I can't save a new template.
The New template button is visible to everyone, but only scoped admins, admins and super admins can save templates.
Why can't I edit the last question?
It's the fixed Global rating question that every client survey ends with. You can only change its number of stars.
I edited a template. Will clients who already received it see the change?
No. A survey keeps the questions it was sent with. Only new surveys use the new version.
Someone tried to send a survey and saw "No templates yet".
There is no template yet. An admin must create one first ("You need to create a template before you can send a survey.").
