Create a Team Survey template

Build, edit, translate, tag, duplicate and delete the questionnaires used by Team Survey campaigns, and understand each question type.

A Team Survey template is the questionnaire your participants will answer. You build it once in Team Survey › Templates and reuse it in as many campaigns as you want. Every campaign uses exactly one template.

Before you start

  • You need the Manager creator role or above. Managers and employees don't see Templates in the menu.
  • Any manager creator can create a template. Only the template's creator or an Admin can edit, translate, tag or delete it. Everyone else opens it read-only and can duplicate it.

The Templates page

Go to Team Survey › Templates. The list shows every Team Survey template of your company with these columns:

Column Meaning
Title The template name. Click a row to open it.
Last update When it was last modified.
Tags Labels you attached to organise templates.

Use the search bar to find a template by title, and the filters to narrow the list by Last update, Creator or Tags. The ... menu at the top gives access to View deleted templates.

Each row has its own ... menu:

  • Edit template tags: add or remove tags (creator or admin only).
  • Duplicate: creates a copy you own and opens it in the editor.
  • Delete: moves the template to the deleted templates (creator or admin only).

Create a template

  1. Go to Team Survey › Templates and click New template. A template called "New template" is created and the editor opens.
  2. In the header card, fill in the template title, an optional description (shown to participants at the top of the survey) and an optional video link (a YouTube link is embedded above the questions).
  3. Click the tag icon in the header card to add tags if you want.
  4. Click Add and pick a block type. An Add button sits above the first block and below every block, so you can insert a block exactly where you want.
  5. Type the wording of each question and set its options (see below).
  6. Everything is saved automatically. The status tag in the top bar shows Saving..., then Auto-saved. If it shows Not saved, your last change was refused; check your connection and try again.
  7. Click the eye icon in the top bar to preview the questionnaire as participants will see it.

There is no "publish" step: as soon as the template exists, it can be picked in a campaign.

Block types

Block What it does Settings
Chapter A section title that groups the questions below it. Participants can jump between chapters from the Chapters menu. Chapter name, description
Paragraph A block of explanatory text. Not a question. Text, description
Open question A free-text answer. Question, description, Required
Rating A scale from 1 to the Max rating you choose (2 to 10). Results are shown as a bar chart. Question, description, Max rating, Allow comments, Required
Multi-choice The participant picks one option in a list you define. Results are shown as a bar chart. Question, description, Answer options (use Add answer option), Allow comments, Required
  • Required: the participant can't submit until this question is answered. Required questions show a * after their title.
  • Allow comments: adds a comment box under a Rating or Multi-choice question, so the participant can explain their answer.
  • Chapters and paragraphs are never answered, counted or charted.

Note: Older templates may contain True/False questions. They still display and can still be edited, but new True/False questions can't be added.

Reorder, copy and delete blocks

Each block has icons to move it up, move it down, duplicate it or delete it. When a template has chapters, a side panel lists them so you can scroll straight to one.

Translate a template

If your participants speak different languages, you can add translations. Participants then switch language from the Translate menu of their survey.

  1. Open the template and click the translate icon in the top bar. The Template translation window opens.
  2. The first row is the Source language, the language you wrote the template in.
  3. Click Add Language and pick a language.
  4. For each language, use the action icons:
    • Translate with AI: fills in the translation automatically. The status shows Translating with AI... while it runs.
    • Download translation: downloads an Excel file with the source text, so a translator can fill it in.
    • Upload translation: imports the completed Excel file (.xlsx only).
    • Delete language: removes the translation.
  5. Each language shows a status such as Translated, Translation missing, To update (the source text changed since the translation was made) or Error.

Important: A campaign freezes its questionnaire, including its translations, when it is launched. A language you add after launch isn't offered to that campaign's participants.

Editing a template that is already used

Launching a campaign takes a snapshot of the template. Editing the template afterwards changes future campaigns only, never a campaign already launched (including scheduled ones). On the campaign's Participants tab, the Template column shows the snapshot date after Version:.

Delete and restore a template

  1. In the ... menu of the row, click Delete and confirm. Deleted templates are stored for 30 days. After this period, they are permanently deleted.
  2. To get one back, open the ... menu at the top of the page, click View deleted templates, then restore it. You can also delete it permanently from there.

A campaign draft that points to a deleted template can't be launched: pick another template in the Template step.

Common questions

I don't see Templates in the Team Survey menu.

You need the Manager creator role or above. Ask your admin to change your access level. If you don't see Team Survey at all, the module isn't enabled for your company.

The template opens but I can't change anything.

The top bar shows Read only: you're not the template's creator and you're not an admin. Use Duplicate from the list to get your own editable copy.

Can a participant choose several options in a Multi-choice question?

No. A Multi-choice question accepts a single option. To collect several choices, ask several questions or use an open question.

I fixed a typo in the template, but participants still see the old wording.

Their campaign was launched before your change and uses the frozen version. The fix applies to campaigns launched from now on.

Can I use the same template in Client survey?

No. The two products have separate template libraries. Client survey templates are managed in Client survey › Templates. See Create a client survey template.
